Insurance

We currently accept the following insurance plans:

 

  • Medical Mutual Ohio

  • United Behavioral Health/Optum Network

  • Aetna

  • Anthem/Blue Cross Blue Shield

  • Cigna

  • OSU Primecare

  • Caresource (Medicaid only, we do not accept Caresource Marketplace)

  • Meritan Health Option
     

We strive to make every effort to assist clients in making our services affordable. All fees are due at the time services are rendered! We accept cash, checks and all major credit cards. Ask about our sliding fee scale to help off set costs. 

 

Insurance Suggestions:

 

We recommend that clients using insurance contact their insurance provider regarding their mental health coverage.  Knowing your insurance benefits and health plan guidelines are important.   It is your responsibility to identify what your deductibles, co-payments, and coverages are for mental health services. Some insurances require a pre-authorization for outpatient services.  You can obtain this via the same number on the back of your insurance card and give them the name of the therapist you are seeking to schedule an appointment with and Key Counseling and Consultation. Please bring your authorization and insurance card with you to your first visit.

 

Check your coverage carefully and find the answers to the following questions:

  • What are my mental health benefits?

  • Can I get a pre-authorization number for my visits?

  • What is my deductible and how much has been met this year?

  • What is the amount of my co-payment per therapy session?

  • How many therapy sessions does my plan cover?

  • How much does my insurance pay for an out-of-network provider?           

 

 

Fees: (Private pay rate if insurance will not be used)

 

Initial Diagnostic Session: $175.00

 

Individual: $125.00

 

Couples/Families: $275.00 for the initial diagnostic session and $225.00 for each additional session

 

Cancellation Fees:

 

We schedule appointments so that each patient has the time and attention he or she deserves. We understand that sometimes a client is unable to make a scheduled appointment due to unforeseen circumstances. However, we do require clients to cancel appointments within 24 hours of a scheduled visit. Missing an appointment prevents us from giving our clients the care they need. It is also detrimental to our practice because it prevents us from scheduling another client who needs care as well. When appointments are missed, our therapists sit idle. 

 

No call-no shows and same day cancellation are subject fees.

 

All fees must are due at the time of services rendered and prior to rescheduling your next appointment. That includes cancellation fees and balances not covered by your insurance provider to include deductibles and co-payments/co-insurance. If you arrive to your scheduled appointment and you have an outstanding balance, you will be required to settle your account prior to being seem by your therapist. Failure to do so will result in the cancellation of your appointment.
